Job Description

GENERAL PURPOSE:

Under the supervision of the Outreach Program Manager (OPM), the Outreach and Enrollment Specialist establishes vital links between Ampla Health providers and individuals in the community. The Outreach and Enrollment Specialist processes applications for health insurance coverage and other benefit programs as well as provides informal community-based health related education throughout Ampla Health’s service area.

MAIN RESPONSIBILITIES & DUTIES:

  • Perform patient education, translation and Insurance/Program Application Assistance (Covered California, Medi-Cal/CalFresh, Connect to Care , Rx Assistance Program, Every Woman Counts, Breast Cancer Detection Program and Medicare Supplements) supported with detailed documentation in the Medical Record
  • Counsel and inform the patient population about the medical and dental services and all other programs available to them through Ampla Health.
  • Assist in the scheduling of patients for services at clinics throughout the area including making arrangements for patient transportation.
  • Provide follow-up care coordination related to insurance enrollment, enabling Programs and/or billing challenges.
  • Refer patients to enabling services based on Social Determinants of Health (SDOH) such as housing application assistance, food bank referrals etc.
  • Bring patient concerns to the attention of the Site Administrator and/or the Patient Navigator.
  • Maintain the confidentiality of patient records in accordance with HIPPA regulations.
  • Act as a liaison between Ampla Health and various community organizations.
  • Notify OPM of potential outreach events and attend community events, meetings/coalitions/taskforces and conduct presentations as assigned.
  • Solicit donations from community agencies and businesses.
  • Coordinate National Health Center Week and Binational activities as host.
  • Attend weekday, evening and weekend events hosted by Ampla Health and community agencies.
  • Attend monthly Outreach Team Meetings and OPM recommended trainings to update knowledge and skills.
  • Attend classes in linguistic and cultural competency.
  • Complete monthly activity tracking spreadsheet daily.
  • Document services provided to patients/clients in Electronic Medical Record System.
  • Distribute current flyers when they become available and remove outdated flyers from the clinic on the 1st and 15th of each month.
  • Other duties as assigned by supervisor

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES:

  • High school graduate or equivalent
  • At least two years’ experience in community social service activities preferred
  • CPR Certified
  • Covered California Certified Enrollment Counselor required
  • Medicare Counselor Certified per State Guidelines
  • Connect to Care Certification through County Medical Services required
  • Completion of training on Prescription Assistance Program, Every Woman Counts and Breast Cancer Detection Program
  • Ability to travel to other locations as needed
  • Valid California Driver’s License and proof of insurance
  • Reliable method of transportation for required travel
  • Excellent communication skills
  • Bilingual preferred
  • Ability to relate to and work with people of all ages, social, cultural and ethnic backgrounds
  • Demonstrate clear knowledge of Ampla Health’s clinic structure, standards, procedures and protocols
  • Computer Literate (Microsoft Office)

WORKING CONDITIONS & PHYSICAL REQUIREMENT:

Works well with patients in a generally comfortable environment office. Employees must possess the following physical requirements:

  • Must be in good health and able to lift up to 40 lbs. and push up to 100 lbs. (on wheels)
  • Must be able to work outdoors in warm or cool situations
  • Must be able to hear staff on the telephone and those who are served in person, as well as speak clearly to communicate information to clients and staff
  • Must be able to read memos, forms, and other pertinent written materials
  • Ability to operate standard office equipment such as a computer, telephone, fax machine, copier, etc.
